INSTRUCTIONS:  Complete this form for each All-Star team.  Each All-Star team must consist of a minimum of nine (9) players and a maximum of twelve (12) players.  Incomplete forms will not be accepted.  Attach a tournament team photo to this roster and return it by the All-Star Roster deadline.  Keep a copy of this form to for your records.  You must take a copy of this form along with original birth certificates for each player to the tournaments.  The tournament director will check each birth certificate of players listed on this form prior to your first scheduled game.  List the name that is on the birth certificate.  Photocopied birth certificates will not be accepted.
